Application for a Disability Study Award

The Disability Study Awards are aimed at encouraging and supporting students as they complete their qualifications.

Please read the information on this application form carefully before submitting it.

How to apply

Complete the form below to apply for this award. 

Please include evidence of your disability, and briefly describe how this award will assist you to complete your qualification.

Criteria

Applicants must:

  •  be currently enrolled at Open Polytechnic.
  • have successfully completed a Level 3 certificate, or at least 3 courses at Level 4 or above with Open Polytechnic.
  • provide evidence of your disability.
  • be citizens or permanent residents of New Zealand.
  • agree to the Terms and Conditions in this application form.

Closing date

Applications close on 30 June. Successful applicants will be notified by 31 July.

Referees

Provide the names of two referees who can be contacted by the Disability Study Award Panel to support your application and comment on your personal qualities.

 Referees cannot be family members, but could be:

  1.  a tutor or lecturer
  2. a church or community leader
  3. an employer or supervisor of, for example, unpaid or voluntary work, vacation, part-time, or full-time employment
  4. a colleague or friend

List the full name, email address and day-time phone number for both referees, and detail their relationship to you.

Terms and conditions

1. Previous Disability Award recipients, past and current Shona Butterfield Scholarship recipients and students receiving Open Polytechnic financial assistance are ineligible for this award.
2. Late or incomplete applications cannot be considered.
3. Successful candidates must meet all course/programme requirements and comply with Open Polytechnic’s regulations where applicable.
4. Open Polytechnic reserves the right not to grant any Disability Study Awards should the panel decide there are no suitable candidates.
5. All decisions are final and correspondence will not be entered into.
