Changes to your Legal name

If you would like to change your legal name or your name has different capitalisations, macrons or special characters that you would like printed on your certificate, make the changes in MyOP (if you can), provide the details below and then send us the evidence.

Change to your legal name: After making the change in MyOP, email a verified copy of the relevant legal documentation to Verifications@openpolytechnic.ac.nz or post your verified document(s) to Open Polytechnic Verifications, 3 Cleary Street, Waterloo, Lower Hutt 5011.
Change to capitalisations, adding macrons or special characters: To request these changes please specify the details below and in an email to academicrecords@openpolytechnic.ac.nz

We can accommodate most requests on your award certificate, however, changes to the Academic Transcript and most other documentation are not currently possible.

Delivery

We will courier your certificate and academic transcript to you. A signature is required for delivery.
Please ensure your delivery address is correct or update it in MyOP before you submit this form.
Your couriered award will be sent within 8 weeks of receiving your request and any verified documents if changes to your legal name were requested.
