Use this form to apply to be awarded a degree level qualification.

Graduation Ceremony – Important information

To attend a graduation ceremony, you must submit this form before 28 February (the closing date).

If you are also graduating with a Diploma, you need to also complete the 'Application for Graduation - Diploma or Graduate Diploma.'

If you have studied courses at another institute towards this qualification, you need apply to have them transferred onto your academic record with Open Polytechnic. If you have not already done this, complete and return the Application for Cross-Credit or Credit-Transfer. 

If you have studied courses for your degree at more than one Te Pūkenga institution, the institution at which you completed most of your study will award your qualification.

Do you wish to attend a graduation ceremony?

If you chose Yes, we will send you confirmation of your eligibility to graduate, and a copy of your Academic Transcript showing your award. You will receive your degree at one of our graduation ceremonies. Information and the Graduation Ceremony Requirements Form will be emailed to you closer to the ceremony date.

If you chose No, we will courier your certificate and academic transcript to you – a signature is required for delivery.
Please ensure your delivery address is correct or update it in MyOP before you submit this form. Your couriered award will be sent within 8 weeks of receiving your request and any verified documents if changes to your legal name were requested.

Changes to your Legal name

If you would like to change your legal name or your name has different capitalisations, macrons or special characters that you would like printed on your certificate, make the changes in MyOP (if you can), provide the details below and then send us the evidence.

  • Change to your legal name: After making the change in MyOP, email a verified copy of the relevant legal documentation to Verifications@openpolytechnic.ac.nz or post your verified document(s) to Open Polytechnic Verifications, 3 Cleary Street, Waterloo, Lower Hutt 5011.
  • Change to capitalisations, adding macrons or special characters: To request these changes please specify the details below and in an email to academicrecords@openpolytechnic.ac.nz

We can accommodate most requests on your award certificate, however, changes to the Academic Transcript and most other documentation are not currently possible.
