Funeral Directing Employer Workplace Agreement Form

The Open Polytechnic requires the details of the applicant's employer, and those of a  verifier for the applicant to progress.

 

The verifier is the person who will verify work-based study tasks that may make up some of the assessments and will be the person with whom the Open Polytechnic can discuss the learner’s enrolment status and academic progress.

Employer responsibilities:

1. Provide opportunities for workplace experience for the student in order that s/he can complete the assessments in the programme.
2. Clearly communicate the workplace’s policies, procedures and practices and other reasonable directions to ensure safe practice
3. Ensure verification and/or supervision is provided for the student completing programme components where there is a verification and/or workplace supervision requirement. The Verifier is required to hold a recognised Funeral Directing Qualification.

Workplace verifier

Learners are required to have a suitable workplace verifier or verifiers. It is preferred that the Verifier has a recognised Funeral Directing qualification at Level 5.

The workplace verifier should be someone who can verify that all workplace assessment is an accurate account of what has been completed in the workplace and who can say that all practical work meets the requirements of the workplace. It could be a manager, or a supervisor.

Note: Workplace verifiers are not required to provide training nor are they required to carry out workplace assessments. Teaching and assessment is the responsibility of Open Polytechnic. The Open Polytechnic will contact the verifier if there are any questions or concerns about the work of a learner.
