Graduation Ceremony Requirements with payment

Conditions and Important Notes: Please read the information below carefully and update all required fields before submitting.

1. Login and check your details:

   
   If your ‘mail to’ postal address or phone number has changed, please update your records in My Open Polytechnic and on this form.  This is the address we will send your graduation pack and guest tickets to. Login and check your details:

   If your full legal name is incorrect, please provide a verified copy of official documentation with your correct legal name, to verifications@openpolytechnic.ac.nz before the close off date for applying to attend graduation. 

2. You may bring up to 8 guests. If you would like to have extra guests attend, indicate this in the form below and we will advise you whether we can accommodate your request after we have finalised all applications.
 
3. Your academic regalia is arranged by Open Polytechnic. Enter your measurements in centimetres. If you require an extra large gown please indicate in the 'Height in cms' box. If your qualification award requires a trencher, use this link to see how to measure your head circumference.
 
4. Payment is required before we can process and send out your graduation pack. After submitting the form you will be taken to a payment details page. Payment can be either by credit/debit card or bank transfer. Fees are based on the items of academic regalia required for your qualification. You will see the applicable fees under the heading ‘Graduation regalia’ on our website. 
 
5. Refunds will be provided if an event causes the cancellation of the ceremony.

As part of the graduation ceremony, your first name and surname will be announced as you receive your certificate on stage. We make every effort to ensure that graduates’ names are pronounced correctly. If there’s information that will help us with your name pronunciation, share this with us below*:

To note, requests of more than 8 guests will be considered once all applications have been received.

*If you are receiving a diploma and have previously been awarded a degree, you can choose to pay the graduate diploma regalia fee and wear a trencher to acknowledge your higher academic achievement. If you choose to do this, please provide the details of your qualification:
