You must work for the organisation that the learner is employed with and be in a position that is responsible for approving the learners study pathway. If you are not, please do not complete this form.

Employer responsibilities:

1. Provide opportunities for workplace experience so that the learner can complete assessments and meet the required workplace hours for each course.
2. Clearly communicate employer policies, procedures and practices and other reasonable directions to ensure safe practice.
3. Ensure verification and/or supervision is provided for the learner where they are completing programme requirements that require verification and/or workplace supervision.

Workplace verifier

Learners are required to have a suitable workplace verifier who has a recognised health and wellbeing qualification or equivalency and/or has had experience in providing health and wellbeing services relevant to the course the learner is applying for. The workplace verifier will monitor the work of the learner, to verify that they have met workplace standards in the work they describe and that their written work is a true reflection of the work they have done.

The workplace verifier should be someone the learner reports to, who can verify that all written work is an accurate account of what has been completed in the workplace and who can say that all practical work meets the requirements of the workplace. It could be a manager, team leader, registered nurse, learning and development person, team coach or a supervisor.

Note: Workplace verifiers are not required to provide training nor are they required to carry out workplace assessments. Teaching and assessment is the responsibility of the Open Polytechnic. The Open Polytechnic will contact the verifier if there are any questions or concerns about the work of a learner.
