Programme structure

The New Zealand Certificate in Long-Term Health Conditions (Level 3) 55 Credits is made up of five individual courses. These non clinical, community focused courses can be studied separately as micro-credentials, or if learners wish to complete all five micro-credentials they may credit these towards the full certificate.  

The certificate programme and its micro-credentials are designed for people working in non-clinical health, disability, or community support roles. Ākonga may also be supporting whānau at home, providing care and assistance for someone living with a long-term condition.

The five courses/micro-credentials are:

  • HWB308 Understanding Long-Term Health Conditions/ MC5360 Long-term Conditions: Risk, Prevention and Support (Micro-credential)
    • On completion of this course or micro-credential, learners/ākonga will be able to: Determine the scope of long-term conditions, management strategies, and the concept of person-centred care. Recognise approaches to privacy and confidentiality for a person with a long-term condition. Identify hauora and cultural considerations for a person with a long-term condition. Recognise common risk behaviours for a person with a long-term condition. Outline health promotion strategies used for managing long-term conditions in Aotearoa New Zealand.
  • HWB309 Understanding Arthritis/MC5356 Long Term Conditions: Risk, Prevention and Support of People Living with Arthritis (Micro-credential)
    • On completion of this course or micro-credential, learners/ākonga will be able to: - Determine the key characteristics of arthritis. - Discuss the prevalence of arthritis in Aotearoa New Zealand. - Identify strategies and medical treatments used to manage pre- and post-diagnosis of arthritis. - Recognise potential long-term impacts of an arthritis diagnosis for a person, their whānau, and caregivers.
  • HWB310 Understanding Diabetes/ MC5357 Long-term Conditions: Risk, Prevention and Support of People Living with Diabetes (Micro-credential)
    • On completion of this course or micro-credential, learners/ākonga will be able to: Determine the key characteristics of diabetes. Discuss the prevalence of diabetes in Aotearoa New Zealand. Identify strategies and medical treatments used to manage pre- and post-diagnosis of diabetes. Recognise potential long-term impacts of a diabetes diagnosis for a person, their whānau, and caregivers.
  • HWB311 Understanding Heart Disease/ MC5358 Long-term Conditions: Risk, Prevention and Support of People Living with Heart Disease (Micro-credential)
    • On completion of this course or micro-credential, learners/ākonga will be able to: Determine the key characteristics of heart disease. Discuss the prevalence of heart disease in Aotearoa New Zealand. Identify strategies and medical treatments used to manage heart disease. Recognise potential long-term impacts of a heart disease diagnosis for a person, their whānau, and caregivers.
  • HWB312 Understanding Stroke/ MC5359 Long-term Conditions: Risk, Prevention and Support of People Living with Stroke (Micro-credential)
    • On completion of this course or micro-credential, learners/ākonga will be able to: Determine the key characteristics of stroke. Discuss the prevalence of stroke in Aotearoa New Zealand. Identify strategies used to prevent stroke and reduce harm from stroke. Recognise potential long-term impacts of stroke for a person, their whānau, and caregivers.

The New Zealand Certificate in Long-Term Health Conditions (Level 3) Qualification overview:

The purpose of this certificate is to provide the health and wellbeing sector with people who have a theoretical and contextual knowledge of common long-term health conditions across diverse populations in Aotearoa New Zealand. Graduates of this qualification will be able to contribute to the support of people living with long-term conditions by promoting health and wellbeing outcomes as part of a multi-disciplinary team, working under supervision as defined by their scope of practice, as well as gaining foundational knowledge to inform their support as a caregiver.
